For your convenience, we have made available here the forms you would otherwise be asked to fill out when you arrive for your appointment. We think you'll find that you can speed through the registration process if you choose to print these out and complete them in advance. Please pay special attention to the question on the Registration Form asking, "Is this a Preventive Medicine Care Visit". You'll find it in bold letters between double asterisks (**) in the middle of the page.
The reason we call your attention to this is that the answer can affect whether or not your insurance carrier will pay for the visit.
Many insurance carriers will pay 100% for any preventive care, which, in our case, means an annual skin check. However, these same insurers may only honor this benefit if nothing else is done (or discussed) at that visit. If you answer "yes", our providers may not be able to perform any additional services at that time. You will need to return for another visit to have anything suspicious removed or biopsied. We really regret this, but we also don't want you to have to pay out-of-pocket for all or a portion of your visit. And that can happen. Should the visit encompass anything but a total body skin exam, you may be held personally responsible for the costs. Please check with your insurance company to learn if this is applicable to you and whether or not co-pays or deductibles apply. We cannot be held responsible for knowing the detailed requirements of individual policies. And we certainly do not want you to have any unpleasant surprises. So, please understand your healthcare policy.
Also, please do not forget to bring all your insurance cards and your referral if required by your insurance carrier.
When a referral is required, you'll want to make sure you have contacted your Primary Care Physician (PCP) well in advance of your scheduled appointment. Up to 7 days notice may be required to allow time for them to issue a referral. Your PCP may request your appointment date in order to process the referral. You may also wish to contact your insurance carrier to confirm that a referral has been issued.
According to federal regulations intended to prevent insurance/identity fraud, we are now also required to ask you for some form of photo ID. We apologize for the additional inconvenience.

For this reason we have begun a new initiative to ensure that each of our patients has a complete skin exam at least once yearly - possibly even more often, if you have a history of skin cancer. The rates of skin cancer have increased dramatically over the past thirty years, even among teenagers. Today, it is estimated that one in every five Americans will develop some form of skin cancer in their lifetime. A complete skin exam is fast, costs nothing additional, and can save your life. So please, help us to help you. When you are offered a gown, please accept it.
